What information will I need to bring for my appointment?
Verifcation of employment
You must bring your last 2 months consecutive check stubs with you. If you get paid weekly, bring the last 8 consecutive check stubs you have received. If you get paid biweekly, bring the last 4 consecutive check stubs you have received. If current check stubs aren't available adn income verification form may be picked up at the office to be completed by your employer.
What if I was recently hired or just starting my job?
If you were recently hired, or just starting your job, you will need to pick up an income verification form at your local childcare office to have completed by your employer.
What if the child(ren's) other parent lives in my home, do I need verification fo their income?
Yes! You are required to bring in the same income verification that is required for yourself. (This includes a step-parent or co-habitiation parent living in the home.)
What else do I need to bring with me?
  • ID information for Parent(s) Responsible Adult - Driver's License or Picture ID
  • Birth Certificates for Child(ren) - Hospital verification or current immunication certificate can be accepted.
  • Current immunization certificates for all children enrolled in care.
  • SS cards for all household members.
  • If attending school, bring copy of current class schedule.
  • Proof of child support if receiving or paying (if applicable).
  • Proof of disability of spouse (if applicable).
  • If child is special needs, bring verification (i.e. SSI award letter, physician statement, etc.)
  • Proof of any other income in the home (SS, SSI, Disability, etc.)
What do I need if Family Support or Protection & Permanacy is authorizing my childcare?
You will need to contact your local Family Support or Protection and Permanancy caseworker or Frankfort at (800)421-1903. All questions will need to be directed to those offices.
What type of child cre provider can I use?
You may choose from licensed or certified care. You may also use a family member or friend who completes the reistration process. The person registering as the provider cannot be a parent ot the child, must verify that they are at least 18 years of age and have a working telephone in the home where the care is provided. The registraion process includes a Criminal Records Check, TB skin test, CA/N checks for all abult household members. The provider will also be required to complete a three (3) hour training course within 90 days aof enrollemnt. It is best if they come in with you at your appointment date and time to complete thier paperwork to enroll as a provider.
What do I do if I fail to bring in the information needed to complete my application at the appointment date?
You will have 30 days from application date to compete all necessary information and requirements needed for approval. However, no eligibility will be granted during the 30 day period and if you fial to complete within 30 days of application your application will be denied and you will be responsbile for any provider charges.
