Procedures
Arrival at the Center |
|
Each child must be escorted into the classroom by his/her legal guardian or someone 18 years of age or older. Children cannot enter the building unsupervised. Upon arrival, all children must be appropriately dressed. Good personal hygiene should be observed. |
Transportation |
|
All children transported by Model City buses must be escorted to the bus by the legal guardian and escorted from the bus by the legal guardian. With written permission, a person other than the legal guardian (who is eighteen years of age or older) may escort a child (ren) to or from the bus. Written permission must be provided for school-age children to be transported by Model City for the after school program. The elementary school will see that your child is safely on the bus. |
Procedure for picking up a child at the center |

1. If you, the parent or legal guardian, bring the child to the Center and are unable to pick him/her up, then a signed, written permission must be provided by you prior to the child being picked up by someone else. Any person picking up a child from the Center, other than the parent or legal guardian, must be IS years of age. 2. On the child's Emergency Record (Yellow Paper), there is a section for escorts other than a parent. You can list names in that section to authorize others to pick up your child In case of emergency. These persons will be allowed to pick up your child without permission. However, these persons will be asked for photo identification. This is for your child's protection. No Exceptions! 3. Your child will not be released to person(s) suspected to be under the influence of a controlled substance. 4. Your child will not be released to escorts other than the legal guardian without proper permission. |
Procedure for Accident or Illness |

1. Each child shall be given n general daily health inspection upon arrival. Any Child Showing Signs of Illness Shall Not be Admitted. If a child has a fever (101°F or higher), is vomiting, has diarrhea, has a rash, runny nose which is dark yellow or green in color, or has symptoms of other communicable illness he/she will be immediately isolated from the other children in the isolation room under the supervision of an adult. The parent will be notified and will be expected to pick up the child as soon as possible. 2. If an accident occurs at the Center, the teacher will notify the parent. If the injury is of a serious nature, the parent will be notified and the child will be taken to the child's doctor or a hospital by bus or ambulance. The child's Medical History Record (Yellow Card) must accompany the child to the emergency room or doctor's office. 3. No medication shall be given except as prescribed by a licensed physician accompanied by written consent of the parent or guardian. Therefore, n written request must accompany all over-the counter medication. The request slip has to be dated daily, with the amount and time for each dosage. The permission slip must be signed by the parent or legal guardian and given to the teacher. If your child has allergies of any kind, please let us know. 4. Model City operates on a no-nit policy. In the event of lice or nits being discovered the child(ren) will NOT be able to return until lice and eggs have been removed and a doctor's slip must accompany the returning child. |
Parent-Teacher Conferences |
|
Parents will meet with the teacher at least three times each year to discuss growth and development of his/her child(ren). A conference can be scheduled either at the request of the teacher or at the request of the parent(s). |
Discipline |
|
The only type of discipline used in the Model City Program is re-direction and praise. If a child continually forgets a rule, he/she will be asked to move to another area or sit in time out for a few minutes from the activity already in progress. |
Suspension |
|
Model City reserves the right for immediate suspension or dismissal for any child whose behavior becomes detrimental to the health and safety of other children at the center. Depending on the severity of behavior, determines as to whether child(ren) will be suspended or dismissed. |
Fieldtrips |
|
No child will be taken on a field trip without prior written consent of the parent or legal guardian. Consent forms will be provided by the Center. |
Immunization |
|
All children enrolled at Model City Day Care are required to have a KY State Immunization Certificate with an expiration date BEFORE admittance. These must be updated as required by law; the Center will notify parents when updates are necessary. 1st notification will be given on a yellow form as to when your child's Immunization is due. If necessary n 2nd notification will be given on the yellow form. Thereafter, if n 3rd notification is given which will be on a orange form, your child will not be allowed to return to the Ccnter until your child's updated immunization certificate is returned. |
Food Program |
|
Child Adult Food Program forms will need to be updated by the parent(s) or guardian at the beginning of each school year (August). These forms will be available upon request. You must have n completed, signed & dated form. The first form must be filled out before admittance. |
Parent Volunteers |
|
Parents are encouraged to visit their child's classroom at any time. Parents are also invited to assist the teacher in any class activity. Even working parents have some time during which they can participate in class activities or provide extra help to the teacher. |
Plan of Action for Disasters |
|
In the event of an emergency such as a bomb threat, all children will be evacuated to the Pikeville College Gymnasium until the threat has passed, or children have been picked up by the parents. |
| *No one other thatn Authorized parties can pick their child(ren) up during these types of Emergeny's due to the Safety of the Child(ren). |
Meals |
|
Nursery - will be fed on demand or as needed |
|
Breakfast - is served from 8:00 a.m. - 9:30 a.m. Lunch - is served from 11:00 a.m. - 12:30 p.m. Snacks - are served from 2:30 p.m. 4:00 p.m. If your child has a special diet that needs to be followed, the Center requires a written statement from his/her doctor, for religious affiliation a written/signed statement from the parent or legal guardian. |
